Note: Our juniors must know that golf has rules and responsibilities. This starts with being on time for class. Please have your junior(s) ready ten minutes prior to start time.
I, the undersigned, do hereby for myself, family, guardians, executors or administrators, waive and release any and all rights to claim for damages, injuries, or anything of the like which my child may suffer in connection with participation in the Front & Back 9 Junior Golf Academy. At times, FB9 may videotape or photograph classes and participants to be used in the media and/or marketing materials. I hereby authorize FB9 to reproduce, copy, exhibit, publish, or distribute any and all such videos or photographs containing my child. I have read the above and understand the expectations.
