I, the undersigned, do hereby for myself, family, guardians, executors or administrators, waive and release any and all rights to claim for damages, injuries, or anything of the like which I may suffer in connection with participation in the Front & Back 9 Golf/Junior Golf Academy (FB9). At times, FB9 may videotape or photograph classes and participants to be used in the media and/or marketing materials. I hereby authorize FB9 to reproduce, copy, exhibit, publish or distribute any and all such videos or photographs containing my child. I have read the above and understand the expectations.
